Acting on the basis of Article 29 of the Act of November 6, 2008 on Patients’ Rights (i.e. Journal of Laws of 2020, item 849, i.e.) and § 75 of the Regulation of the Minister of Health of November 9, 2015 on types, scope and models of medical records and the manner of their processing (Journal of Laws of 2015, item. 2069) Krajmed Medical Center informs that due to the expiration of the storage period, individual medical records in the form of medical histories will be submitted for destruction (deletion) in a manner that makes it impossible to identify the data.
It is planned to destroy the medical histories of patients treated between 1994 and 2004.
Those interested in receiving the original of the above-mentioned documentation are asked to submit an application ( download application):
– directly at the reception desk of the Krajmed Medical Center, or
– via snail mail to:

– via email to: [email protected]
Persons authorized to receive medical records are:
– the patient – the person to whom the records pertain,
– the patient’s legal representative,
– the person authorized by the patient.
The deadline for submission is December 31, 2024. After this date, the documentation will be destroyed and it will not be possible to collect it. Collection of the aforementioned documentation is possible after a personal appearance at the Krajmed Medical Center, presentation of an identity document confirming eligibility for collection and a receipt in person, or after submission of a written authorization and a receipt for collection.
